As a Medicare Certified Rehabilitation Facility, HealthTrak provides comprehensive therapies and services to meet the individual needs of all patients, which includes Physical Therapy, Occupational Therapy, Aqua Therapy, and Speech Therapy; along with having a Licensed Clinical Social Worker on staff.

Our goal at HealthTrak is to provide the most comprehensive collaborative care based on the individual needs of the patient to return them to their highest possible level of functioning. Below is a brief description of the types of services we provide

PHYSICAL THERAPY

Our highly experienced licensed staff provide services for a multitude of musculo-skeletal and neurological diagnosis which include and are not limited to hip, shoulder, knee ankle replacements, work and recreational injuries, auto accidents, slip and fall injuries, strokes, back and neck rehabilitation along with any physical therapy needs.

• We provide the highest quality care and treatment using individualized treatment and exercise programs for orthopedic, neurological, pediatric and sports medicine rehabilitation.
  
  
• We use a wide variety of treatment modalities, along with and including our state of the art exercise equipment which includes: BTE, Swim-EX, Biodex, Powerline, and Cybex equiptment.
  
  
• Our facility provides hydrotherapy and is equipped to provide care for hand and foot injuries along with wound care.
  

OCCUPATIONAL THERAPY

Returning to work and patient’s highest level of functioning is the goal of occupational therapy, where patient independence is stressed in treatment. A registered occupational therapist oversees treatment for neuromuscular illness, head injuries, fractures, tendon and nerve damage, work related injuries and other conditions that require treatment.

As one of the few facilities in the area to offer Functional Capacity Evaluations, HealthTrak also uses a work simulator, therapeutic exercise programs, Back School, job site analysis, pre-employment screening and other services to facilitate the patient’s return to work and patient’s highest level of functioning in their lifestyle.

SPEECH THERAPY

A full range of speech and language pathology services is available for adult cases. Our licensed speech and language therapist is fully trained to assess and treat a variety of communication, cognitive, voice and swallowing disorders that are the result of strokes, head injuries or disease.

SOCIAL SERVICES

Along with a comprehensive range of rehabilitation services offered, HealthTrak has a Licensed Clinical Social Worker on staff. The social service goals are as follows:

• To assist patient in maintaining a lifestyle that adequately meets their needs by providing resources and assistance with financial, insurance and transportations issues.
• To assist patient in adjusting to changes in lifestyle due to injury, surgery and changes in health status by providing reassurance, encouragement, grief counseling, emotional support, and redirection to appropriate resources and agencies when needs arise.
