HealthTrak was founded by Jim Eckhout, PT over 28 years ago in 1983. HealthTrak moved to our spacious Bay City Clinic in 1993. Mr. Eckhout is a Mayo Clinic Alumni and has extensive experience in the rehabilitation of post-surgical patients, orthopedic injuries, back and neck rehabilitation, along with sport-related injuries having been a supervisory-level Physical Therapist for over thirty years. As President and owner of HealthTrak, Mr. Eckhout is an active participant in the Tri-Cities professional and Business community and an active member of the Chamber of Commerce.

Our Mission Statement: The basic mission of this rehabilitation agency is to deliver high quality rehabilitative services to the residents of Bay, Saginaw and adjacent counties while providing an environment of excellence in which health care professionals can work. The agency is dedicated to providing a multidisciplinary approach to rehabilitation for the injured, handicapped and disabled. The agency will strive to provide the finest in health care services and programs that will contribute to the physical, psychological, emotional and social well-being of its patients and the community served by the agency.

• To provide an alternate site for rehabilitation services in the community other that the traditional hospital setting.

Our Objectives:

• To render high quality rehabilitation services to patients to control the cumulative disabling effects of prolonged physical and/or mental illness.
• To minimize residual physical disability.
• To help return the individuals to optimum living within his or her capabilities.
• To hasten convalescence and expedite recovery.
• To contribute toward the comfort and overall well-being of the patient.
• To correlate the objectives of this agency to the objectives of the patient’s physician and other health care providers.
• To provide, maintain and train qualified personnel who can achieve the objectives of the agency.
• To assist in helping each patient reach his or her maximum physical independence so he or she may reach an economical independence and assume his or her rightful place in society while learning to live within the limits of his or her capabilities.
• To provide facility hours that would meet the needs of the working public that would otherwise not receive rehabilitation services.
• To provide a cost effective environment for patients to receive rehabilitation services.
